Overview

Flatback syndrome is a deformity of the lower spine in which the normal inward curve of the lumbar region, typically ranging from roughly forty to sixty degrees, is reduced or lost entirely. The spine ordinarily balances the head over the pelvis with minimal muscular effort by coordinating lumbar lordosis with thoracic kyphosis and pelvic orientation. When that lordosis disappears, the trunk shifts forward, a state called positive sagittal imbalance, in which the line dropped from the seventh cervical vertebra falls in front of the sacrum rather than over it.

The body responds with a cascade of compensations: the pelvis tilts backward, the hips extend, the knees bend, and unfused spinal segments above the deformity hyperextend, all to keep the gaze horizontal and the trunk as upright as possible. These adaptations are metabolically demanding and eventually overwhelm the available muscle reserve, so the patient progressively stoops forward. The altered loading pattern also narrows the spinal canal and nerve-exit openings at affected levels, linking the structural deformity to nerve compression and leg symptoms.

What causes it

Historically, the most recognized cause was Harrington distraction instrumentation used for scoliosis correction, which straightened coronal curves while inadvertently eliminating lumbar lordosis. Many longstanding cases trace back to that earlier surgical era. Today the more common iatrogenic cause is lumbar fusion performed without restoring adequate sagittal alignment, particularly multilevel fusions ending at the lower lumbar segments.

Degenerative causes are equally important in older adults. Progressive disc collapse, ankylosing spondylitis, post-traumatic kyphosis, and osteoporotic vertebral compression fractures can all erode segmental lordosis over time. Risk factors therefore span a wide range: prior spinal instrumentation, advancing age, inflammatory and metabolic bone disease, and previous thoracolumbar trauma. Patients with limited compensatory reserve, such as those with hip flexion contractures or weakened paraspinal muscles, are most likely to become symptomatic as the deformity progresses.

Symptoms and warning signs

The dominant complaint is a stooped posture and an inability to stand fully upright. Most patients describe a sensation of falling forward that builds through the day and worsens with walking, reflecting progressive muscular fatigue. Pain is typically located in the low back, thighs, and posterior pelvis (the regions bearing the sustained effort of the compensatory muscles), and many people report needing to flex the hips and knees, lean on a shopping cart, or find a seat to remain upright for any length of time.

Because foraminal and central canal narrowing frequently coexist with the deformity, patients may also develop lumbar radiculopathy or neurogenic claudication: buttock and leg pain provoked by standing and extension and relieved by sitting or leaning forward. Physical examination shows the characteristic posture with pelvic retroversion and knee flexion, and there may be focal sensory or motor changes in a nerve-root pattern.

Certain features require prompt evaluation. Progressive weakness in the legs or feet, saddle-area numbness, or any loss of bladder or bowel control can signal cauda equina syndrome and are a surgical emergency. Fever, night sweats, unexplained weight loss, or a known history of cancer raise concern for infection or a tumor as the underlying cause and should prompt immediate medical attention.

How it's diagnosed

Diagnosis combines a thorough clinical assessment with standing full-length radiographs. Weight-bearing thirty-six-inch lateral and posteroanterior films of the entire spine and pelvis are the cornerstone study; they allow measurement of lumbar lordosis, thoracic kyphosis, the sagittal vertical axis, and pelvic parameters including pelvic incidence, pelvic tilt, and sacral slope. A significant mismatch between pelvic incidence and lumbar lordosis is central to characterizing and grading the deformity. Because supine imaging can mask the deformity entirely, weight-bearing studies are essential, and dynamic (flexion-extension) films help assess how flexible or rigid the curve is.

MRI is obtained to evaluate the spinal cord, nerve roots, and the degree of foraminal and central canal narrowing that explains radicular or claudicant leg symptoms. CT provides fine detail of bony anatomy, prior fusion masses, and any suspected pseudarthrosis. Radiographic findings are always interpreted against the patient's symptoms (asymptomatic degenerative changes are common), and laboratory studies or contrast-enhanced imaging are added when infection or malignancy must be excluded.

Treatment options

Management begins with non-surgical care. Structured physical therapy focuses on hip-flexor flexibility, core and extensor strengthening, postural training, and the appropriate use of gait aids. Analgesics, anti-inflammatory medications, and targeted treatment of contributing osteoporosis are used alongside therapy. When associated foraminal or central canal stenosis drives radicular leg pain, selective image-guided epidural or transforaminal steroid injections can provide meaningful symptom relief, though injections address the nerve irritation, not the underlying spinal malalignment.

When surgery is considered

Surgery is considered when the deformity is fixed, disabling, and has not responded adequately to a sustained course of non-operative care, or when progressive neurologic compromise develops. The overarching operative goal is to restore lumbar lordosis and rebalance the spine over the pelvis.

This is most often achieved through spinal osteotomies, bone cuts that realign the spine. A Smith-Petersen osteotomy, a wedge-shaped cut through the posterior column (the posterior elements), is used for more flexible deformities. A pedicle subtraction osteotomy is a three-column osteotomy that removes a wedge of bone through the full width of a vertebra, and it is reserved for rigid or sharply angular curves that require greater correction. These osteotomies are combined with decompression of any compressed neural elements and long-segment instrumented fusion to maintain the corrected alignment. Because these are major reconstructive procedures carrying substantial complication risks, careful patient selection, optimization of bone quality before the operation, and precise surgical planning of correction targets are all essential. When cauda equina syndrome or a rapidly progressive motor deficit is identified, urgent surgical decompression takes priority over staged reconstruction.

Frequently asked questions

Can flatback syndrome improve without surgery?
Conservative care, including physical therapy for hip flexibility and core strengthening, pain medication, and treatment of osteoporosis, can meaningfully reduce symptoms and help patients function better. These measures address the muscular and symptomatic burden of the deformity; they do not correct the underlying loss of lumbar lordosis. Patients whose deformity is fixed and disabling, or who develop progressive neurologic problems, are those most likely to need surgical correction.
Why does the discomfort tend to get worse as the day goes on?
The back muscles, pelvis, hips, and knees all work harder than normal to keep the trunk upright against the altered alignment. Early in the day those compensatory muscles can manage, but as they fatigue the trunk tilts further forward, pain increases, and many people find they need to lean on a shopping cart or sit down to find relief.
