Overview

The sacroiliac (SI) joint connects the base of the spine, the sacrum, to the pelvic bones (iliac bones) on each side. It is a strong, partly cartilage-lined joint that transfers load between the spine and the legs, stabilized by powerful ligaments and the surrounding muscular envelope. Because the SI joint sits immediately beside the lower lumbar spine and refers pain into the buttock, posterior thigh, and groin, its symptoms overlap substantially with those of lumbar disc disease, facet arthropathy, and lumbosacral nerve compression. This overlap is precisely what makes SI joint dysfunction one of the most important non-spine sources of low back and buttock pain.

Studies suggest SI joint pain accounts for roughly 15–30% of chronic axial low back pain presentations below the L5 level, and it is a condition that clinicians must actively consider before attributing pain to the spine.

What causes it

Pain can arise from inside the joint itself (intra-articular pathology such as osteoarthritis, inflammatory sacroiliitis, infection, or crystalline disease) or from the surrounding ligaments and capsule, a presentation sometimes called sacroiliac dysfunction.

Risk factors for mechanical SI joint pain include:

  • Prior lumbar spine surgery, especially fusion extending to the sacrum, which shifts increased load onto the SI joint
  • Pregnancy and the postpartum period, when hormone-driven ligamentous laxity reduces joint stability
  • Leg-length discrepancy or gait asymmetry, which creates uneven stress across the joint
  • Trauma such as a fall onto the buttock or a motor vehicle collision
  • Repetitive shear loading from certain occupational or athletic activities

Inflammatory sacroiliitis is a distinct entity. It is a defining feature of the spondyloarthropathies (notably ankylosing spondylitis and related conditions), which tend to affect younger adults and are associated with the HLA-B27 gene variant. Older patients and those with degenerative change more commonly present with osteoarthritic SI pain.

Symptoms and warning signs

The classic presentation is one-sided pain below the L5 level, centered over the dimple above the buttock cheek (the posterior superior iliac spine). Patients can often point precisely to where it hurts, a finding clinicians call a positive Fortin finger test. Pain may spread into the buttock, groin, and back of the thigh but characteristically does not run below the knee in a true nerve-root pattern, which helps distinguish it from lumbar radiculopathy. Symptoms are typically aggravated by transitional movements: getting up from a chair, climbing stairs, or standing on one leg.

On physical examination, a cluster of provocation maneuvers, including the thigh thrust, compression, distraction (gapping), Gaenslen test, and the FABER (Patrick) test, can help confirm the SI joint as the source. Three or more positive tests substantially raise that likelihood. Unlike a true nerve-root problem, neurological examination is typically normal, the straight-leg-raise test does not reproduce radicular tension, and there is no frank dermatomal deficit.

Certain features should prompt urgent evaluation rather than continued conservative management. Seek immediate care if you develop fever, night sweats, or rest pain that does not ease (which may indicate joint infection); unexplained weight loss or a history of cancer with new or worsening symptoms; numbness in the groin or inner thighs (saddle area), or any loss of bladder or bowel control, which require emergency assessment to exclude cauda equina pathology; or rapidly worsening leg weakness.

How it's diagnosed

Diagnosis is clinical and confirmatory rather than purely radiographic, because imaging findings frequently do not correlate with symptoms.

Imaging studies each serve a different role:

  • Plain X-rays of the pelvis can show sclerosis, erosions, joint-space narrowing, or ankylosis, and are useful for screening for spondyloarthropathy.
  • MRI is the most sensitive test for active inflammatory sacroiliitis, demonstrating bone marrow swelling, capsulitis, and erosions. It is also essential when infection or malignancy is suspected, as it reveals abscess, marrow replacement, or destructive lesions.
  • CT best characterizes cortical erosions and chronic structural changes.

Laboratory testing, including inflammatory markers, HLA-B27, and cultures when infection is being considered, supports the workup when inflammatory or infectious causes are possible.

Diagnostic injection is the reference standard for confirming a mechanical SI source. An image-guided injection of local anesthetic directly into the joint that produces meaningful, reproducible relief of the patient's typical pain supports the SI joint as the pain generator and is the key step in separating it from a lumbar spine source. This distinction is critical, because the two conditions require fundamentally different treatment approaches.

Treatment options

Treatment is directed by the underlying cause.

For mechanical SI joint pain, management begins conservatively with activity modification, analgesics, and non-steroidal anti-inflammatory medications. A structured physical therapy program emphasizing core strengthening, pelvic stabilization, correction of leg-length discrepancy, and (in selected patients) a pelvic (SI) belt to reduce excessive joint motion forms the foundation of care.

When conservative measures are insufficient, image-guided intra-articular corticosteroid injection can provide diagnostic confirmation and, in some patients, meaningful (though often temporary) pain relief. Radiofrequency ablation of the lateral sacral branches offers longer-lasting denervation for patients who respond well to diagnostic blocks.

For inflammatory sacroiliitis, treatment follows rheumatologic principles: NSAIDs, physical therapy, and disease-modifying or biologic agents such as TNF or IL-17 inhibitors. Infectious sacroiliitis requires prompt antibiotics and sometimes surgical drainage. Malignant involvement is directed by oncologic care. This diversity of causes underscores why accurately separating SI joint pain from a spine source, and from other mimics, changes management entirely.

When surgery is considered

Minimally invasive SI joint fusion is an option for patients with well-localized pain that has been confirmed as arising from the SI joint by concordant diagnostic blocks, and who have not improved after exhausting nonoperative care. Evidence supporting SI fusion in carefully selected patients continues to accumulate. Surgery is reserved as a last step (it is not a first-line treatment) and is only considered after conservative and interventional options have been fully explored.
